5 years and over: to TNW Communitas on ERS (who re-book in ENT if needed)
Adults audiology: audiology form on EMIS via email
<16 children’s audiology: email audiology using EMIS form (who triage to Communitas when appropriate, who do not always send results to GPs) – both services can resolve wax in order to assess
Urgent and emergency ENT
Adults: ERS (referral, urgent, otherwise speficied, RAS)
<16 years: letter via email to bartshealth.paediatricentpccs@nhs.net

The NHS Discharge Medicines Service (DMS) is a new nationally commissioned essential service that all community pharmacy contractors have to provide. The aim of the service is to improve the communication of changes to a patient’s medication when they leave hospital, to prevent harm from medicines, reduce readmissions and to improve patients outcomes. Trusts (Acute, Mental Health, Community) will identify appropriate patients and refer them to their usual or preferred community pharmacy on discharge with information about medication changes made in hospital.
Community pharmacist have been commissioned to provide a new NHS Discharge Medicine Service, which is being rolled out locally across Tower Hamlets, Newham and Waltham Forest (TNW). A joint working group has been working on the implementation and the service will be rolled out gradually from all Trusts across North East London (NEL) from July 2021.
These are legacy formularies which have been superseded by the fully published North East London Joint Formulary -netformulary https://www.nel-jointformulary.nhs.uk/ . The legacy formulary information should not be used as these are no longer kept up-to-date.
North East London (NEL) is in the process of harmonising the existing formularies in-use across NEL onto a single NEL-wide electronic formulary. The new NEL electronic formulary is expected to be launched by the end of 2024.
Users can access the full version of the City & Hackney/Homerton formulary, however, this formulary has not been updated since April 2024 and will not be updated further.
Please refer to the NEL Formulary and Pathways Group (FPG) decision log for formulary updates made since October 2022.

REMINDER: Change to referring to the Homerton Allergy Service
Dear Practice Managers and GPs,
Homerton Hospital has asked us to send a reminder about the change in its allergy service as they are receiving few referrals.
Please remember that referral to Homerton Allergy service is via Advice and Refer option only on eRS – please let practice referral admin teams know as appropriate. You can continue to refer to the service but, in many cases, you may get management advice rather than conversion to a referral. It is therefore important that you include a full history and details of any treatment already undertaken over what time periods.
