City and Hackney JPG approved the use of rotigotine in February 2017.
The JPG recommended the use of rotigotine to be initiated by Parkinson's Disease (PD) specialist in the following indications:

  • Where the oral route is temporarily, intermittently or permanently not available or reliable.
  • To prevent acute akinesias or Dopamine Withdrawal syndrome (to be reviewed by PD nurse/ consultant 1 month after initiating for this indication).
  • Used in combination with other medications in order to treat motor-fluctuations or levodopa-induced dyskinesias.
  • As a trial agent before more invasive therapies.
HUHFT PD team have agreed to review all patients that they have initiated on rotigotine within 4-6 weeks of initiation.

North East London Formulary & Pathways Group's Decision - opicapone for Parkinson's disease

February 2024 - The North East London Formulary & Pathways Group (NEL FPG) approved the use of opicapone for treatment of Parkinson's disease as part of NEL formulary harmonisation. This medicine should only be prescribed in primary care following recommendation by a specialist. Opicapone is recommended as an alternative catechol-O-methyltransferase (COMT) inhibitor for patients where use of entecapone is inappropriate, ineffective or not tolerated.

Codeine should only be used to relieve acute moderate pain in children older than 12 years and only if it cannot be relieved by other painkillers such as paracetamol or ibuprofen alone. A significant risk of serious and life-threatening adverse reactions has been identified in children with obstructive sleep apnoea who received codeine after tonsillectomy or adenoidectomy:

  • in children aged 12–18 years, the maximum daily dose of codeine should not exceed 240 mg. Doses may be taken up to four times a day at intervals of no less than 6 hours. The lowest effective dose should be used and duration of treatment should be limited to 3 days

  • codeine is contra-indicated in all children (under 18 years) who undergo the removal of tonsils or adenoids for the treatment of obstructive sleep apnoea

  • codeine is not recommended for use in children whose breathing may be compromised, including those with neuromuscular disorders, severe cardiac or respiratory conditions, respiratory infections, multiple trauma or extensive surgical procedures

  • codeine is contra-indicated in patients of any age who are known to be ultra-rapid metabolisers of codeine (CYP2D6 ultra-rapid metabolisers)
  • codeine should not be used in breast-feeding mothers because it can pass to the baby through breast milk
  • parents and carers should be advised on how to recognise signs and symptoms of morphine toxicity, and to stop treatment and seek medical attention if signs or symptoms of toxicity occur (including reduced consciousness, lack of appetite, somnolence, constipation, respiratory depression, 'pin-point' pupils, nausea, vomiting)

April 2023 - The North East London Formulary & Pathways Group approved the use of lozenges for acute vaso-occlusive pain management, in sickle cell patients attending the Emergency Department or the Haematology Day Unit, who are on an oral pain regime. May 2022 - The JPG agreed to approve brivaracetam for the treatment of pharmaco-resistant focal and generalised epilepsy. This would be restricted for initiation by/or under the recommendation of a consultant epileptologist or specialist epilepsy nurse. Patients should only be transferred to GP for prescribing once they are on a stable dose and tolerating treatment. Prescribers should ensure that patients are not prescribed concomitant levetiracetam.
