BHR – Palliative and End of Life care available out of hours

The Marie Curie Rapid Response Overnight nursing service, which was introduced as a pilot scheme in February 2022, ended on 31 March 2024.

People requiring end of life support will continue to have access to the local overnight district nursing services and a 24/7 nurse led advice line (for patients, carers and professionals), along with other Marie Curie services that remain unchanged. This means that Marie Curie continue to provide planned care at home overnight in Barking, Havering and Dagenham, while Redbridge planned overnight care is provided by the community palliative care team provided by the London Borough of Redbridge. Marie Curie also provide a support line, which is available for anyone who needs it with trained support line officers who can offer practical, financial, and clinical information, and emotional support.

BHR – DVT proforma and NOAC guidelines

The new DVT referral proforma is now accessible in patient mergeable format on EMIS templates. This can be found under ‘DVT Referral Form BHR v1’.

The DVT proforma should be printed out, once completed, and given to the patient to take to the anticoagulant appointment (which should also be booked by calling ext 6216).

Frailty Virtual Ward – BHRUT

BHRUT launched their Frailty Virtual Ward on Monday 11 September 2023.

Virtual Wards are a safe alternative to NHS bedded care for patients who would otherwise be in hospital. Being cared for at home reduces some of the risks associated with hospital admissions and helps prevent loss of independence for frail patients.

The Frailty Virtual Ward team is currently taking referrals from within Queens or King George Hospital for patients who can benefit from an earlier discharge with Virtual Ward support. 

BHRUT will shortly be working with their colleagues in community care, residential care homes and LAS to take referrals for patients where an admission to hospital could be avoided. They will be working with primary care for future expansion.

  • Social worker
  • Pharmacist
  • Support staff

BHRUT are working closely with NELFT and integrated community teams to ensure patients can be safely discharged with the support they need.

Discharge Medicines Service – Community Pharmacy 

The NHS Discharge Medicines Service (DMS) is a new nationally commissioned essential service that all community pharmacy contractors have to provide. The aim of the service is to improve the communication of changes to a patient’s medication when they leave hospital, to prevent harm from medicines, reduce readmissions and to improve patients outcomes. Trusts (Acute, Mental Health, Community) will identify appropriate patients and refer them to their usual or preferred community pharmacy on discharge with information about medication changes made in hospital. 

Community pharmacist have been commissioned to provide a new NHS Discharge Medicine Service, which is being rolled out locally across Tower Hamlets, Newham and Waltham Forest (TNW). A joint working group has been working on the implementation and the service will be rolled out gradually from all Trusts across North East London (NEL) from July 2021.
