Information-Community Medical Paediatrics
 

Child Development Team-
offers diagnostic management, assessment and on-going support of any child who may have a disability or developmental difficulties.
 

Eligibility/Age Criteria : Children 0-19 year old who live in Waltham Forest.

Multi-disciplinary involvement from specialist nurses, paediatricians, therapy services, and special educational needs (SEN) early years services ensures regular assessment of the child’s needs as required.

Referral Information:
Referrals will be accepted from parents and guardians and professionals in health, education and social care. A specialist children’s service referral form must be completed and signed consent from parent/guardian to share information must be provided.  Referrals can be emailed or posted directly to the service or made through single point of access (SPA) please ensure that sections 5 and 7-11 are completed.

Eligibility/Age Criteria:

0-19 Children and young people with asthma may also be referred to the Asthma Nurse Specialist. This is done by GPs and hospital Specialists.

About the self-service criteria

WF Specialist Paediatric Continence Nurse Service-Tier 2 Referral
(assists parents / carers with the management of a child or young person with night wetting once they are over five years and daytime wetting from the age of four years until their 18th birthday)

Referral/Access criteria
The service is accessible to children and their families registered to a GP in the Borough of Waltham Forest, or attending school within the borough, already toilet trained who are: 
5 years and older with night wetting 4 years with daytime wetting 4 years with both day and night wetting, Previously dry children / young person who begin to wet or soil themselves already toilet Children with sever constipation, overflow soiling
Please note if no evidence of 3-6months of Tier 1 support has not been offered referral will be rejected.
