High-risk’ Referral Criteria

• Hospital admission or ≥ 2 exacerbations of wheeze/asthma in the past 6 months (this includes courses of steroids prescribed in primary care)
• Evidence of Salbutamol over-use (>twice a week / >6 inhaler devices a year)
• Poorly controlled symptoms (ACT <19, school missed, exercise limitation)
• BTS Asthma Management Step 4 or above
• Parent/professional concerns about compliance
• Missing lots of time off school due to asthma
• Children 1 year of age till 18^th birthday

Service Information

We are a specialised team of nurses dedicated to supporting children and young people with asthma, wheeze, allergies, and eczema. Based at The Bridge, Mile End, we are available Monday to Friday, 8:00 AM to 6:00 PM (excluding bank holidays).

Our focus is on helping families confidently manage these conditions at home, reducing the need for hospital visits. We provide personalised care, advice, and support to ensure your child stays safe and well.

Using a personalised asthma action plan, we work with families to empower children and young people to manage their asthma, wheeze, and related conditions effectively. We also collaborate with GPs, health visitors, school nurses, and hospital specialists to provide comprehensive and well-rounded care.

Our goal is to support your family every step of the way, helping your child stay healthy and thrive at home.
