Fraudulent prescriptions

The dentistry, optometry and pharmacy commissioning team for London has been made aware of a few cases of fraudulent prescriptions attempting to be filled at pharmacies across London.
These have been for Codeine Phosphate 15mg/5mL – 400mL.
The attached incident memoranda are being shared with community pharmacists across London, and GP practices should also be aware.
