Follow-up Communication – NEL Dementia Referral Form

Further to the communication published in December regarding the launch of the standardised NEL Dementia Referral Form, please see the attached follow-up communication.

As planned, the referral form is now live across both EMIS and SystemOne for all NEL boroughs. This communication serves as a reminder to practices and encourages those that have not yet adopted the standardised referral form to do so for new referrals to Memory Services.

The refreshed communication also reflects the wider programme of work to improve equity of access, consistency of care and patient experience across dementia pathways in North East London.
