Stroke Support Toolkit for London

The Stroke Association has published a practical Toolkit to help GPs and practice teams signpost stroke survivors, carers and families to appropriate support.

It brings together information about the Stroke Support Helpline, online and peer support, information resources, communication support, emotional wellbeing, fatigue, benefits, and support for carers.

The Toolkit is designed to answer a simple question: “I’ve had a stroke, where can I turn for help?

Please share it with GPs, nurses, care coordinators, social prescribers, reception teams and other colleagues who may support people affected by stroke.
