Please remember to attach the referral form to all fertility referrals on e-RS.
The form (attached below) can be located on the following platforms:
- GPs | BHR Hospitals
- On the DoS (Directory of Service on e-RS)
- NEL GP Portal
Please ask your practice team to ensure this is available for you to use and save this to your clinical systems for ease.
