Shared Care Template Letters

Here are examples of letters that will be used across NEL for shared care purposes.

This is the letter that secondary care will be sending to GP practices to confirm shared care:
Shared care REQUEST letter – specialist to primary care (NEL)

Here are letters that primary care can send back to secondary care.
Shared care ACCEPTANCE letter – primary care to specialist (NEL)
Shared care REFUSAL letter – primary care to specialist draft (NEL)
