PCN extended Access Q4 2023/24 claims

Please be reminded that this email as a reminder to complete and submit the attached for the Q4 2023/24 provision for the Extended Access DES.

The deadline for submission is 30 April 2024.

Please submit completed claim forms to the claims inbox on nelondon.primarycareclaims@nhs.net with the borough name and PCN name in the subject email field.

A polite reminder to ensure the declaration section at the bottom of the claim form is also completed.
