BHRUT – GP radiology request form guidance

The BHRUT radiology request form has been shared previously, but we know that not all surgeries can access the updated forms and we get a large number of queries relating to incorrect radiology request forms, so we have listed some points below to help practices.

Please ensure that the highlighted areas/sections are adequately filled to avoid potential delays:

  • Correct Lead GP to ensure the report is assigned correctly.
  • Practice Address to differentiate surgeries.
  • Practice code to enable electronic transfer of the report.
  • Request Form signed by the GP as mandated by Radiation Regulations (electronic signature can be accepted with clear GMC code).
  • Request Form correctly dated and no longer than 3 months.

We need the form in .pdf format (not .tif format)

Direct Access for General X-Rays

The radiology services at BHRUT operate a direct access service only for general x-rays requested by a GP and it’s important that;

  • the patients have a correctly completed form for their arrival and
  • are made aware of the correct times to come in.

This will avoid wasted journeys for patients.

The times are on the back of the Radiology Request Form.

The service is currently trying their best to avoid sending the patient back to the GP, hence we advise the patient of what needs changing/adding to the form, and the e-mail address the GP should send it back to so they can relay this to the GP practice over the phone whilst at the hospital.

This can only work when the practice is available to take the call and the amended form can be sent over by email there and then.

We have an increasing number of patients telling us that the practice is refusing to add the Practice code/GMC number/date/patient address and re-send the form. Patients have even been told that we don’t need them to make the amendments or told they can add the information to the form themselves.

All of which delays care to the patient. It often frustrates and sometimes angers the patient.

We thank you for your ongoing co-operation with this process and ensuring that the forms are correctly completed, and the information given to our patients is correct.
