NEW Replacement FIT Kit request form for Bowel Cancer Screening

This email concerns the process of requesting replacement FIT test kits from the regional Bowel Cancer Screening Programme (BSCP) Hub.

The London Bowel Cancer Screening Hub has issued an updated form to request replacement FIT Kit Tests, this will be in effect from October 2026, and the previous form will be decommissioned.
Using the updated form reduces barriers, supporting patients to access screening. Crucially, it includes a section for your team to notify us of any Additional Care Needs (ACNs) your patient has; meaning we can provide vital reasonable adjustments for your patients. These reasonable adjustments will be logged and maintained on our system, so reasonable adjustments can be offered throughout the screening pathway and in future bowel screening episodes for the patient.
To ensure safe processing and compliance with clinical data governance, all requests must use the attached template. Please note the following mandatory changes to our workflow moving forward:
- Mandatory Form: From the 1st October 2026; the Hub will no longer process the outdated request form and only the attached, updated form will be accepted (Test Kit Request Form).
- Test Kit Request Form: Please complete the attached form for all subjects within your practice who require a replacement kit at that time. To reduce administrative pressure, we have included the option to add multiple patients if needed, but the form can be submitted for just one patient – depending on who needs a replacement kit at that point in time.
