Tower Hamlets New Provider: Community Anticoagulation Service
Dear colleagues,
Following a review and open tender, NHS North East London (NEL) has been implementing a new single Community Anticoagulation Service (CAS) model across north east London, which is being delivered by four new providers.
For the development of this new single CAS model, we worked with local providers and clinicians and sought feedback from patients to carefully evaluate our current services. The aim was to develop a service to minimise the historic variation in the way we provide and manage community anticoagulation services across north east London and offer value for money.
Our new service reflects best practice and adheres with national guidelines to provide equitable, accessible, high-quality care across all north east London by:
- providing community-based care, ensuring patients receive Warfarin monitoring and medication advice, while improving clinical outcomes
- undertaking medication reviews to optimise anticoagulation therapy
- providing domiciliary visits for eligible housebound patients.
WF Federated GP Network Limited is the provider who will deliver community anticoagulation services across Tower Hamlets from 1st April 2026.
WF FedNet are contacting patients to arrange appointments.
