West Ham United Foundation PCN programme
The West Ham United Foundation PCN Programme works with local NHS Primary Care services, to enable young people to make a positive connection with an empathetic, positive and caring mentor. We can receive referrals from Havering, Barking & Dagenham, Redbridge, Tower Hamlets, and Newham. We can support young people aged between 11-18 years old. Sessions are once a week/fortnight for up to six months.
Primarily an early intervention, prevention service, mentors support young people who could be at risk of being a victim or perpetrator of violence & exploitation, or other social and emotional hurdles:
- Vulnerable/affiliated to violence knife crime and exploitation
- Victim/perpetrator of crime
- Bullying
- Family/friend within the criminal justice system
- Experiencing/impacted by domestic abuse
- Autism, ADHD, SEMH, SLCN diagnosis/need
- Disengaging from school/risk of exclusion/NEET
- Anxiety/Mental Health need
- Key moments of transition
Sessions are 121 in the community/school/GP Practice and can offer young people the opportunity to engage in open dialogue to seek help and support, without them feeling judged. We also look to the future, exploring future aspirations, self-esteem and positive mindset change, whilst helping to champion short term change in their lives such as school engagement, positive relationships and healthy routines.
