Second line choice after iontophoresis for HDSS score 3-4
Third line choice after topical aluminium and iontophoresis for HDSS score 1-2 

100-250-units per focus.  Max dose 300-400 units per treatment. Reconstitute each vial with 1-4mLs normal saline or 1% lidocaine if being used on an area associated with high pain.

Clonidine Patches are for specialist initation only within the hypertension clinic. These are used for the  treatment of hypertension where the oral route is inappropriate or there is a lactose allergy to tablets. Doses are initated and titrated within clinic.

Clozapine supply is monitored in the UK by one of three main companies depending on the Brand used.  Service users should not have a supply made or drug administered  without a current blood test showing showing a 'GREEN' result.   Users may be on a weekly, fortnightly or monthly testing scheme. If patient has missed a dose within 48 hours a supply may be made by ELFT (patient should be transferred from a Psychiatric Consultant to an ELFT one) but if longer than this time then re-titration from  12.5mg dosage is usually required with Consultant and Pharmacist involvement.

Aspirin

Should not routinely used for primary prevention of cardiovascular disease.

Enteric coated aspirin should not be used as it has a poor cost:benefit ratio. There is no convincing evidence that it reduces gastrointestinal bleeding (Drug Ther Bulletin, Jan 1997, p.7-8) and it has a lower bioavailability vs dispersable.

Gastroprotection with lansoprazole is advised.

CODEINE PHOSPHATE

Second line for mild to moderate pain-'add on' treatment to paracetamol (+/- NSAIDs)

Also used for GI surgery 'short bowel' patients-requests for this indication should be annotated accordingly

Note: Injection is subject to controlled drug storage and register requirements

Codeine Lintus 15mg/5ml for cough

The therapeutic indication is as an anti-tussive in a non-productive cough.

Morphine sulphate can be used intrathecally at low doses for pain relief e.g. 200-300mcg.  It can take 2-3 hours to take effect necessitating IV morphine as well.

It is also available as a 30mg in 10mL high viscosity ampoule which is inserted into a pump and slowly delivered over approximately two months.  This is for Consultant use only e.g. Neurosurgery

Clonidine tapering of dose

Clonidine is used for agitation in critical care areas. 
Once a patient has been transferred from critical care to a ward, clonidine should be weaned off as tolerated, with the aim of discontinuing it before discharge from hospital.  
The timescale depends on the duration of clonidine therapy, as follows:
Less than one week: 24-48h
One week to one month: 5 days
More than one month: 2 weeks

Pregabalin - Lyrica brand for neuropathic pain

 

 

Pregabalin should only be prescribed for the treatment of neuropathic pain under the brand name Lyrica® (unless there are clinical contra-indications or other special clinical needs e.g. patient allergic to an excipient, branded product unavailable etc which apply to Lyrica®, when you should not prescribe Lyrica® or pregabalin)

2. When prescribing pregabalin for the treatment of neuropathic pain to patients you should (so far as reasonably possible):

a) prescribe by reference to the brand name Lyrica®; and

b) write the prescription with only the brand name "Lyrica", and not the generic name pregabalin or any other generic brand.

3. When prescribing pregabalin for the treatment of anything other than pain, you should continue to prescribe by reference to the generic name pregabalin

Adjunct in partial seizures with or withour secondary generalisation

For treatment of neuropathic pain-see section 4.7.3

Pregabalin

Prescibe under the brand name Lyrica for neuropathic pain unless clinical contraindications or other special needs exist (see guidance from NHS England)

Second line for neuropathic pain if amitriptyline/gabapentin ineffective or patient experiencing unmanageable side effects

Licensed drug and approved for use as adjunctive anti epileptic therapy where all other options have failed for partial onset seizures with or without secondary generalisation in adult and adolescent patients with intractable epilepsy.  The intention is to slowly increase dose and replace therapy of a previous medication.
10mg, 25mg, 50mg, 75mg, 100mg tablets for oral administration
10mg/mL oral solution, 10mg.mL solution for injection /infusion
Starting doses of 10mg once daily increaed to twice daily in two weeks time, then 25mg twice daily to a potential maximum of 100mg twice daily.
Exclusions:

  • children under age of 16 years
  • CKD patients on dialysis
  • Galactose intolerance
  • Pregnancy unless clinically necessary i.e. benefit to mother clearly outweighs risk to foetus

Monitor for neutropenia.

Fenfluramine is recommended as an add‑on to other antiseizure medicines for treating seizures associated with Dravet syndrome in people aged 2 years and older, only if:

  • seizures have not been controlled after trying 2 or more antiseizure medicines

  • the frequency of convulsive seizures is checked every 6 months, and fenfluramine is stopped if it has not fallen by at least 30% compared with the 6 months before starting treatment

Pregabalin - Lyrica brand for neuropathic pain

 

 

Pregabalin should only be prescribed for the treatment of neuropathic pain under the brand name Lyrica® (unless there are clinical contra-indications or other special clinical needs e.g. patient allergic to an excipient, branded product unavailable etc which apply to Lyrica®, when you should not prescribe Lyrica® or pregabalin)

2. When prescribing pregabalin for the treatment of neuropathic pain to patients you should (so far as reasonably possible):

a) prescribe by reference to the brand name Lyrica®; and

b) write the prescription with only the brand name "Lyrica", and not the generic name pregabalin or any other generic brand.

3. When prescribing pregabalin for the treatment of anything other than pain, you should continue to prescribe by reference to the generic name pregabalin
