Transfer of Care

DOACs can be prescribed in primary care under transfer of care following recommendation from specialist

 https://gp.walthamforestccg.nhs.uk/management/medicines/prescribing/sharedcareguidlines/barts-health.htm

If it is for the specific condition you are asked to continue treatment for and you are happy to prescribe in accordance with the guideline, then take over the agreed responsibilities including prescribing. Please ensure the document is scanned into the patient's electronic records.

If sufficient information has not been provided and if you do not feel happy to continue to prescribe, it is advisable to refer the patient back to the specialist clinic and complete and send as soon as possible an 'inappropriate prescribing request communication form' .

Bezafibrate- when should it be used

The NICE guideline on familial hypercholesterolaemia recommends that adults with the condition who have intolerance or contraindications to statins or ezetimibe should be offered referral to a specialist with expertise in this condition for consideration for treatment with a bile acid sequestrant, a fibrate or nicotinic acid to reduce their LDL-C concentration. The decision to offer treatment with a bile acid sequestrant, a fibrate or nicotinic acid in addition to initial statin therapy should be taken by a specialist with expertise in familial hypercholesterolaemia. Healthcare professionals should exercise caution when adding a fibrate or nicotinic acid to a statin because of the risk of muscle-related side effects (including rhabdomyolysis). Gemfibrozil and statins should not be used together.
