Cervical Sample Taker Questionnaire 2025 – Reminder
Thank you to those practices/PCNs who have returned the cervical sample taker questionnaire already to CSL.
To date CSL have only received 32 completed questionnaires out of 315 (Practices & PCNs) from NEL.
I am e-mailing therefore to remind all practices and PCNs to complete and submit the cervical sample taker questionnaire and return to CSL before the deadline.
Please return the completed questionnaire to: hsl.csl.cstd@nhs.net by Friday 28th November 2025. I have attached the questionnaire to this e-mail for ease of access.
