Webinar – Care Coordination and the Plan for Neighbourhoods

The NEL Personalised Care Programme Team is delighted to invite you to our NEL-wide webinar: Care Coordination and the Plan for Neighbourhoods: Enhancing community health through collaborative care efforts on Thursday 4th December 2025 from 12:00 – 13:00.

We recognise the vital importance and role played by Care Coordinators in primary care and neighbourhoods to support the needs of our local populations.

Our early work has identified some great examples where Care Coordinators are having a real impact. We also know there is more work to do around role definition and further embedding the role in primary care. There has been an expansion in the Care Coordinators in post across North East London, increasing from 50 to 236 in the past 3 years so the time is right to come together to focus on the future of this role in NEL.

This webinar will cover:

  • Understanding Care Coordinator Role within Neighbourhoods
    • Explore the Care Coordinator’s role in improving practice efficiency and supporting local neighbourhood working.​
  • Supporting Population Health
    • Learn how Care Coordinators contribute to population health management and reduce healthcare system pressures.​
  • Lived Experience Insights
    • Gain insights into the needs and challenges faced by Care Coordinators in their daily work following the recent Care Coordinator training and follow up survey
  • Embedding Care Coordination
    • Identify practical steps to integrate Care Coordination into local delivery models effectively.
