Waltham Forest GP Update: Early Management of Common MSK Conditions
The Waltham Forest Community MSK Service has seen a significant increase in referrals following restrictions to direct MRI requesting.
Many referrals are being received within the first six weeks of symptom onset. Evidence and NICE guidance support conservative management as the first-line approach for most non-traumatic MSK conditions, as many patients improve within 6–12 weeks through advice, activity, exercise, self-management and appropriate analgesia.
MRI is generally not indicated unless there are red flag symptoms, significant neurological deficits, suspicion of serious pathology, or failure of appropriate conservative treatment. Early imaging can lead to unnecessary anxiety and interventions, as findings often reflect normal age-related changes rather than the source of symptoms.
Where clinically appropriate, consideration should be given to a period of conservative management before referral to specialist MSK services.
