Update from Barts Medical Examiner’s office regarding the new MCCD (Medical Certificate of Cause of Death)

Dear Colleague,
It appears that the new MCCDs only have a unique identifier on the first page. The national medical examiner office has asked DHSC to rectify this on the next print run.
Medical Examiners need to be confident that the two pages (back and front) of the MCCD relate to the same patient.
The suggestion from the regional medical examiner team is for the unique identifier to be written by the attending practitioner on the second page when signing where possible.
We would be grateful if this is taken onboard to prevent any delays to after death processes.
