Universal Care Plan Programme update

The Universal Care Plan Programme has received reports from users who were unaware of the data exchange functionality of the programme. With this in mind they have produced a ‘did you know’ sheet:

The Universal Care Plan (UCP), a personalised care planning application available across London, exchanges information with your EMIS and TPP SystmOne Electronic Health Record (EHR) systems, reducing the need to duplicate entry from Universal Care Plan to EHR. The list of fields that are exchanged are available here for EMIS users and here for TPP SystmOne users. 

What this means is that when you input new or update certain information in either your EHR or the Universal Care Plan, the next time the Universal Care Plan is opened, the new information will flow into and replace the previous, now out of date information in the other system. This exchange is done using the Valida client and will only happen if the UCP is launched from Valida

Importing of information from the Universal Care Plan to your EHR will create a new encounter. 

Prognosis field data exchange:

If any prognosis category is entered in the Universal Care Plan this will be imported as a Gold Standard Framework (GSF) coded entry into your EHR (GSF prognostic indicator Stage A (blue) years plus; Stage B (green) months; Stage C (yellow) weeks; Stage D (red) days). Any of these Gold Standard Framework coded entries in your EHR may automatically add the patient to your local End of Life Care (EoLC) register. If your patient has been coded to have a prognosis of “years” then you may feel that your patient is not appropriate to be on the EoLC register and therefore you should manually remove them from your list.
