Tackling unplanned care with personalised clinical coaching in North East London

In an exciting new collaboration, NHS North East London (NEL), UCLPartners, and Health Navigator (HN) are working together to identify patients most at-risk of emergency hospital stays, and provide clinical coaching to help them avoid visits to A&E.

Patients identified, with the help of AI, will be offered personalised clinical coaching with Health Navigator qualified coaches. This initiative helps reduce the burden of unplanned care on GPs, without increasing their workload. Qualified clinical coaches work directly with patients to educate them, improve care plan adherence, empower them to self-manage their conditions and coordinate with services, leading to fewer hospital admissions and less reliance on emergency services. This ensures that primary care teams can focus on proactive, efficient care, improving patient outcomes while alleviating pressure on busy practices.

This work is set to transform care for those at risk of unplanned hospital visits, helping both patients and the healthcare system in North East London thrive.

A phased roll out of this initiative started in north east London in November, with City and Hackney, Waltham Forest, Havering, Redbridge and Newham already on board. We now plan to start the roll-out in Barking and Dagenham on 23 October, and the project team will be sharing more information at the Barking and Dagenham PTI today (5 August).

Printed versions of the patient information resources (attached to this email) shared with GP practices in the coming weeks, and online versions are on our NEL GP information portal.
