NHS Diabetes Prevention Programme – expansion of referral routes for people with a history of Gestational Diabetes Mellitus

To support improved access to the NHS DPP, the programme will now accept self-referral for people with a history of GDM, in addition to the usual referral route from General Practice, without the need for a glycaemic test result to be submitted.
The self-referral pathway is supported by a national NHS England GDM webpage through which individuals will be able to contact their local NHS DPP provider by completing a short form or via email or telephone.

- FAQs for healthcare professionals in General Practice and maternity services
- A patient information sheet which can be shared with people with a history of GDM and who are eligible for the programme, which will direct them to the NHS England GDM self-referral webpage. A designed leaflet and translated versions will be available to download from the DHSC Campaign Resource Centre (please note these have not yet been added but will be available shortly). Systems may choose to produce their own localised leaflets using the national leaflet as a template.
