Next Havering Place based Partnership Showcase event: BHR Frailty Virtual Ward

Everyone is welcome to attend the next in our series of Havering showcase events, focussed on Virtual Wards, will provide another opportunity for leads from the Havering Place based Partnership to update you on the work that is ongoing which continues to bring partners together and improve outcomes for local people. The presentation will focus on the new frailty virtual ward service, that is being launched in September. This service aims to support our elderly and frail populations with care and management at home for conditions that they would have otherwise received care in hospital for. It is expected that this will lead to better outcomes for the target population and improve hospital performance.
