Havering Place based Partnership Showcase event: PACT household model which is being initiated by Liberty PCN

Following on from our successful series of showcase events where leads from the Havering Place based Partnership have updated you on the work that is continuing to bring partners together and improve outcomes for local people, you are cordially invited to the next event, which will shine a spotlight on the PACT household model which is being initiated by Liberty PCN.  There will also be an update on the Ageing Well Service, operating from St George’s Health and Wellbeing hub and the frailty line.

Speakers include:

  • Dr Mary Burtenshaw, Clinical Care Lead, Urgent Care and Emergency Care;
  • Dr Simon Green, Associate Medical Director, Integrated Care for Frailty Consultant Physician and Geriatrician, BHRUT and NELFT,
  • Kirsty Boettcher, Assistant Director, Age Well and Care
  • Luke Burton, Director, Joint Director of Partnerships, Impact and Delivery, North East London ICB and London Borough Havering.

This event will provide the opportunity for you to ask questions and provide your views to continue to shape our work.
