New standardised dementia referral form and pathway launched across North East London

We are pleased to announce the introduction of a new standardised dementia referral form and pathway across North East London. This initiative is designed to provide GPs with a clear, consistent process to connect patients with memory services.

The approach, piloted successfully in City & Hackney last year, has been developed collaboratively with input from:

• Mental health clinical leads across all seven NEL boroughs
• Local Medical Committees (LMCs)
• Mental health steering groups

This shared referral form and pathway marks an important step in ensuring consistency, improving patient experience, and strengthening collaboration across the system.

The RP NEL Dementia referral form V1 is now live on both EMIS and SystmOne and available for use while the official go live date is 1st of December. We encourage all GPs to adopt this process from the 1st of December 2025, to ensure consistency and timely access to memory services.
