Actions required by all PCNs to continue NHSmail access

PCNs need to complete the below form and arrange for it to be signed by a board member by 15 March. After this date, any accounts hosted in the ICB container will be marked as leavers. The signed form should be returned to tim.jameson@nhs.net asap and by 15 March.
