C&H – A look at the Transitional Pain Clinic at Homerton

In this short video, The Homerton Inpatient Pain team wants to give you a brief summary of their new Transitional Pain Clinic. The clinic is a telephone one and reviews patient that are discharged with newly prescribed strong opioids or gabapentinoids with the aim to support their weaning  process. They want to give you an overview of the clinic’s  structure and your feedback is obviously invaluable.
