Dear Practices,
As you are aware, the majority of patients requiring DOACs, will have their DOAC treatment managed in primary care.
On occasion it may be necessary to refer a complex patient on/requiring DOAC treatment to Homerton Anticoagulation Team, for haematology input/review.
If this need arises, please refer via e-RS (see below) and not by email.
