The NHS Digital Weight Management Programme could help you if you are living with obesity and also have diabetes, high blood pressure, or both. If this is you, you could benefit from this 12-week programme. It’s free and easily available via a smartphone, tablet, or computer.
There are three levels of support available, depending on your preferences and needs.
- Level 1: access to digital content only
- Level 2: access to digital content plus 50 minutes of human coaching over the 12 weeks
- Level 3: access to digital content plus 100 minutes of human coaching over the 12 weeks
The eligibility criteria are listed below.
You must be:
- age 18+
- diagnosis of diabetes (type 1 or type 2) and/or hypertension
- BMI of 30+ (or =27.5 for BAME groups)
- You must have a smartphone, tablet, or computer with internet access
You must not:
- have severe/moderate frailty as recoded on a frailty register
- be pregnant
- have a diagnosed eating disorder
- have a weight management programme considered to pose greater risk of harm than benefit*
- have had bariatric surgery in the last two year
- for those individuals aged over 80, the GP will need to provide supporting information that they are suitable for the programme
* For example, patients with chronic obstructive pulmonary disease that are unmanaged, meaning not on medication and/or not subject to regular clinical review or have not completed a current active management programme such as diabetes management or cardiac rehabilitation.
Getting a Referral
Referrals are made by a trained health care professional via the GP surgery. If you meet the eligibility criteria listed above and are motivated to join this programme please contact your GP surgery to discuss the next steps.
