Sign up for Unplanned admissions data project – Urgent call for action

This is a reminder for the final few GP practices to log into DCC and sign the Data Sharing Agreement for the Unplanned Admissions (Falls prevention) project with the London Borough of Barking and Dagenham (LBBD).

This initiative is designed to support your patients aged 65+ who are at high risk of falls by integrating health data with social care insights. By enabling front line Local Authority colleagues to offer proactive, preventive support, this project will reduce the likelihood of patient crises, helping them stay healthy and independent, and reducing unplanned hospital admissions.

Participation from all practices is essential to ensure that all eligible patients can benefit from this proactive care, ultimately reducing pressures on GP services by enabling local authority teams to provide targeted interventions.
