Diabetes Results (Type2) National Diabetes Audit 2024-25

Thank you Primary Care! Our achievement caring for people with type 2 diabetes; results of National Diabetes Audit for 2024-25

NEL is an outlier!  In a good way!

Review of the 2024-25 National Diabetes Audit (Type 2) shows that NEL is currently the leading ICS in helping our patients with diabetes achieve the 8 care processes recommended by NICE. We want to extend a huge thank you to all in Primary Care that work to deliver this result – and which helps identify deterioration early and provides the basis for supporting each patient to achieve better control of their condition.

We achieved a completion rate of 73.1% and our close rival, who we swapped positions with from the 2023-24 NDA, North West London ICB were second with 72.2%. The England average was 57.6%.

It’s easy to forget the scale of the work-effort this amounts to – so here is an illustration to help. You know the work tasks, the call/recalls, arrangements for clinics, chasing test results, care planning with actions and follow-up etc. The total type-2 population across NEL is equal to 2.3 West Ham Stadiums (158,405). All of those would have received call-recall and most have some work-up and care planning completed. Of these 1.7 stadiums-worth, 115,860 patients, completed their 8 care-process checks. Picture below to help remind the size of a full stadium of people (West Ham can seat 68,000).

The ICB does invest to achieve this. The current work on a new LTC Outcomes Framework will be looking at this diabetes investment plus other areas but for now it is right to acknowledge and congratulate on the work done.

A big thank you once again – the ICB Executive Management Team

Some further detail from the NDA results 2024-25 with commendations to all:

Ten PCNs achieved over 79% 8CPs complete collectively; these were

All NEL PCNs achieved over 63% (England Average = 57.7%)

Control of diabetes is the aim; the results across NEL are more varied in this area and is where we need to reduce variation.  It is fortunate that one Place, City and Hackney, offers a clue for learning in the ICB to help take us all forward.  City and Hackney have 8 PCNs which are all in the top 10 NEL PCNs for percentage of patients with 3 treatment-targets controlled.  Well done City and Hackney!  And to the two other PCNs from Newham and Tower Hamlets.

England 3TT average is 45%
