– Supporting patients and their families with telephone and face to face contact in the last months of life.
– Supporting in providing advanced care plan discussions and UCP formation.
– Signposting and co-ordinating district nursing teams, community physiotherapy and Occupational therapy teams, hospital beds and fast track care packages.
– Provision of day and night sitting services.
– Support and education for care homes in providing EOL care.

– provide support and advice to health care professionals/GSF meetings
– provide face to face initial assessment and follow up as required for patients with specialist palliative care needs:
- uncontrolled complex symptoms, physical, psychosocial or spiritual distress
- Challenges making decisions or having conversations regarding advanced care planning
- End of life care needs – support in prescribing anticipatory medications
- Short term interventions for those who experience challenges as a consequence of treatment, to improve wellbeing
Triage for community inpatient unit admission to Margaret centre for specialist palliative care needs or preferred place of death in last 2 weeks of life
