The Diabetes Support Service (for those with Type 2 diabetes) is open for self-referral.
Attached, please find template SMS that can be shared with patients to support self-referral – please choose any preferred template. This will also make it easier to encourage sign ups – self-referral might help your practices manage demand for GP and Practice Nurse visits
