BHRUT frailty virtual ward
BHRUT launched their Frailty Virtual Ward on Monday 11 September 2023.
Virtual Wards are a safe alternative to NHS bedded care for patients who would otherwise be in hospital. Being cared for at home reduces some of the risks associated with hospital admissions and helps prevent loss of independence for frail patients.
The Frailty Virtual Ward team is currently taking referrals from within Queens or King George Hospital for patients who can benefit from an earlier discharge with Virtual Ward support.
BHRUT will shortly be working with their colleagues in community care, residential care homes and LAS to take referrals for patients where an admission to hospital could be avoided. They will be working with primary care for future expansion.
The Virtual Ward team consists of
- Consultant and Advanced Care Practitioners
- Nurses
- Therapists
- Social worker
- Pharmacist
- Support staff
BHRUT are working closely with NELFT and integrated community teams to ensure patients can be safely discharged with the support they need.
