Join Our Fourth GP Learning Network Session

The Health Innovation Network South London, in collaboration with the London Ambulance Service, invites GPs working in care homes across London to our fourth GP Learning Network session.

Join colleagues from Primary Care and the London Ambulance Service for an interactive session exploring how collaborative working can support admission avoidance and improve outcomes for care home residents.

This session will showcase practical examples of integrated working between LAS, Primary Care, and Care Home teams, demonstrating how strong partnerships can help reduce unnecessary hospital admissions while ensuring residents receive the right care in the right setting.

Through case studies, discussion, and shared learning, participants will:

  • Explore effective approaches to admission avoidance
  • Learn from real-world examples of collaborative care
  • Strengthen partnerships across services
  • Identify best practice that can be applied in everyday clinical settings

Together, we are building a London-wide network that promotes collaboration, reduces avoidable hospital admissions, and advocates for better care for older adults.
