Join heart failure experts for a practical, case-based session designed for community and primary care colleagues across London. The session will focus on supporting the implementation of the updated NICE heart failure guidance published in September 2025, and the new 2026/27 HF Quality and Outcomes Framework metric (HF009), which aims to ensure patients with heart failure are on all four pillars of optimised therapy.
The session will walk you through the essentials of heart failure, including:
✔️ A simple roadmap for putting NICE guidance into practice – what’s changing and how to make it work in primary care
✔️ Making diagnosis easier with NT‑proBNP – when to test, what results mean, and how it helps avoid missed or delayed HF diagnoses
✔️ Getting HF medications right – clear, practical tips to optimise therapy and improve both longevity and quality of life
✔️ Bringing HF care closer to home – how community nursing, primary care, and specialist teams can work together more seamlessly
✔️ Practical examples from across London – real cases and take‑away tools you can use the next day
