New single community anticoagulation service model launching soon
Dear colleagues,
Following a comprehensive review, we are pleased to announce the introduction of a single, NEL-wide Community Anticoagulation Service (CAS) care model. This new approach will ensure patients across north east London receive equitable, accessible, and high-quality care.
Our review highlighted variations in how anticoagulation services were delivered under the previous model, which relied on multiple community-based providers. To address these inconsistencies, we worked closely with local providers and clinicians, and sought feedback from patients, to redesign services in line with best practice and national guidelines, helping to improve clinical outcomes.
The new CAS model will:
- Continue to provide warfarin monitoring and medication advice within the community
- Include medication reviews to optimise anticoagulation therapy
- Offer domiciliary visits for eligible housebound patients.
After a rigorous tendering process, we are pleased to confirm the selected providers who will deliver community anticoagulation services across north east London in the coming year:
| Community anticoagulation service provided by | Area covered | Go-live date |
| Together First CIC | Barking and Dagenham | January 2026 |
| City and Hackney Integrated Primary Care CIC | City and Hackney | April 2026 |
| WF Federated GP Network Limited | Tower Hamlets | April 2026 |
| Newham Waltham Forest Havering | January 2026 | |
| Healthbridge Direct Limited | Redbridge | January 2026 |
A huge thank you to everyone who contributed feedback and ideas during this process.
We will keep you updated throughout the transition and share details of any changes. Please support this process by working with incoming providers and responding promptly to requests for data sharing agreements to ensure continuity of care for patients.
