BHRUT – Frailty line / Acute medical support line
The Acute Medical Support and Frailty Lines are live and should be used to access BHRUT SDEC, Ambulatory Care, Frailty Virtual Ward and the Urgent Assessment Clinics at St George’s Ageing Well Centre.
Please share the flyers with the GPs in your practices and promote their usage. The response times are very quick, which will benefit GPs, patients, and are an effective alternative to the Emergency Department.

Frailty line
The Frailty Line gives you direct access to a senior geriatrician to discuss whether there is an alternative to hospital admission for frail older patients.
It is the referral point for Frailty Virtual Ward and urgent assessment clinics at St George’s Ageing Well Centre.
Please have patient details to hand including their clinical frailty score.

Acute Medical support line
BHRUT has also launched an acute medical support line to help reduce unnecessary hospital admissions. This will provide consultant level support to identify the most appropriate pathway for patients to follow.
This includes at home care plans or potential referral to hospital services, such as Ambulatory Care, Same Day Emergency Care or the Emergency Department
