Population Health Management Linked Health and Social Care Data
As part of our ongoing collaborative work to integrate health and social care, North East London Integrated Care Board (NEL ICB) is extending a crucial project to enhance our Population Health Management (PHM) capabilities. This involves linking Adult Social Care, GP, SUS, Mental Health, and Community data, to make improvements in population health.
Background
In January 2024, a project under the title ‘Newham Patient Data PHM – BI Assured process’ was approved by the Data Access Group (DAG) and ratified by the Information Governance Steering Group (IGSG). This project focused on linking an agreed set of GP and Acute data with the London Borough of Newham, as an early adopter.
Request
We are now expanding this project to include all NEL boroughs. We are also incorporating the National Adult Social Care Data (CLD), which has recently become available via the National feed. This extension also includes the National Mental Health and Community Datasets.
We have been working with the NEL IG Team and your Data Protection Officer to establish a Data Protection Impact Assessment (DPIA) and Data Sharing Agreement. These were reviewed and recommended for approval by the NEL ICB Data Access Group and approved by the NEL ICB Information Governance Steering Group. We recommend updating your Privacy Notice. We have provided a suggested PN to your DPOs.
Use Case
The primary aim of this project is to enhance our understanding of the health and social care needs of the population by linking and analysing the data sets.
This will enable us to understand relationships between health and social care, which is crucial for identifying and addressing health inequalities. It will also contribute to work to improve prevention, effectively plan resources, reduce system costs, and reduce hospital admissions.
Actions Required
Please take the following actions once you receive the alert from the Data Controller Console
- Log in to the Data Controller Console.
- Review and accept the:
- Data Sharing Agreement (DSA)
- Short Data Protection Impact Assessment (DPIA)
- Update your practice’s Privacy Notice to include this processing.
By accepting the DSA you are responsible for managing data subject access requests and handling any data breaches.
