Helping you prepare for the expansion of the Universal Care Plan

The expansion of the UCP into a personalised care and support plan is planned for December this year.

To help users and front-line clinical staff prepare, the universal care plan programme will be hosting webinars to discuss the upcoming changes to the UCP, explain their impact on current users, and provide an opportunity to address any questions.

In progress forms:

Once the Universal Care Plan has been expanded, all submitted forms will be migrated to the new template. There is a risk that some ‘in progress’ forms will not be migrated, therefore please complete these before the December go-live.  Please note, in progress forms cannot be viewed by urgent care services.
