Call for examples of practice in ICB’s

We are looking for examples of work in local systems – as ICBs, ICP’s and ICS’s which aim to improve the quality and consistency of learning disability annual health checks and health action plans.

What we are looking for

We are looking for examples of work – either in planning stages or in implementation to improvement the quality of any part of the annual health check cycle – this could include;

  • Identifying people who are eligible for a health check but may not be accessing them. For example, people from minority ethnic communities
  • Different models of delivery of the annual health check which support reasonable adjustments – clinics, outreach, etc
  • Health action plan development
  • Population health approaches to addressing risks of poor health, identified through learning disability annual health checks
  • Learning disability “champions” or local quality kitemark

How will the examples be used?

  • The examples will be used to inform the development of an NHS England learning disability annual health check framework.
  • Examples may be used to illustrate key points to support local systems to implement the framework (wording will be checked with systems for factual accuracy)

Developing the learning disability annual health check and health action plan framework

The aim of the framework is to:

Develop a health outcomes focused quality framework for the learning disability annual health check and health action plans.

The objectives are to;

  • Set out a pathway and outcomes focused quality framework for a learning disability annual health check and health action plan
  • Strengthen the links between the content and outcomes of an annual health check through the health action plan, focusing on the identification of risk of preventable or avoidable health issues including health determinants such as ethnicity and age and the individuals overall goals for their health
  • Describe the functions and roles required for the delivery of a high quality annual health check and health action plan pathway which addresses risks associated with premature mortality

The Framework will be developed to reflect the whole annual health check cycle with a clear focus on health outcomes.
