About the Programme
Healthy Living for people with type 2 diabetes is a nationally commissioned structured education (SE) programme initially available until 31 March 2024. The programme is a free of charge, self-directed, web-based programme that employs behaviour change models and nudge techniques to support users to take action to manage type 2 diabetes.
Starting with key essentials of type 2 diabetes, participants ‘unlock’ more content as they progress through each article. Content is delivered in the form of quizzes, articles and interactive tools and some topics include:
- Healthy eating including content tailored for traditional foods from Black African, Black Caribbean, and South Asian ethnic backgrounds.
- Getting enough exercise
- Achieving and maintaining a healthy weight
- Living, working, and socialising with diabetes
- Looking after feet and eyes
- Reducing diabetes related distress, and more.
The Healthy Living programme is delivered in English and participants can translate content using an online tool such as Google translate. This way of translating the content in the programme is in line with other NHS web-based content such as nhs.uk.
The programme is optimised for use with multiple device types.
Eligibility Criteria
Inclusion
- Aged 18+
- Diagnosed with type 2 OR;
- A carer or relative of a person living with type 2 diabetes
Healthy Living is suitable for people who are both newly diagnosed, and who have established diabetes. Participants must have regular access to a smartphone, tablet, laptop or desktop computer and an email address.
Exclusions
- Be living with type 1 diabetes
- Have non-diabetic hyperglycaemia (NDH)
- Be less than 18 years old
- Be on the End of Life (EOL) pathway
- Be pregnant at time of referral
How to refer
Self-referral
Self-referral available online at https://www.healthyliving.nhs.uk/, creating opportunities for direct-to-consumer methods to reach people with type 2 diabetes.
GP referrals
Referrers can signpost users to the self-referral route. A direct referral pathway into the service from GP systems is currently under development. Once available, NHSEI will communicate to Primary Care via Diabetes Regional and Clinical Networks.

- Actions taken by the GP to provide the self-referral webpage (using text, letter, leaflet etc.) can be coded as “referred”.
- Primary Care will need to code activity into the programme when a person is signposted to the Healthy Living.
- The Healthy Living programme will report back to GPs on the status of participants attendance (attended/completed code) to enable patient records to be updated.
